Appointments

  • You must make an appointment in advance to receive services.
  • Appointments can be booked directly by calling 415-409-3456.
  • Appointments made online are only requests. The requested time is not guaranteed until the staff has collected additional information and confirmed the appointment via phone call.
  • A credit card is required to hold an appointment and may be used to charge for no-show or late cancellation fees.
  • Notice must be given at least 24 hours before an appointment to cancel or re-schedule.
  • Arrival in the clinic lobby before or at the time of my appointment is expected.
  • Failure to provide 24 hours’ notice or failure to show for my appointment will result in a charge of the current no-show fee.
  • Arrivals more than 10 minutes late will not be seen, and no-show fees may apply.
  • Recharge Medical recommends that you arrive a few minutes before your scheduled appointment to attend to any needed paperwork.
  • Recharge Medical may refuse to schedule appointments or provide service for any reason.

Payment and billing

  • Payment for all services and fees are due at the time of service.
  • The patient or the patient’s guardian is solely responsible for all charges in full, whether or not care is covered by insurance.
  • Recharge Medical bills only the services and tests performed in their office; fees for outside services are billed separately.
  • Recharge Medical does not bill or accept payments from insurance companies.
  • Recharge Medical does not provide super bills or any insurance billing codes to patients or third parties.
  • Recharge Medical charges your credit card for your services rendered and fees.
  • Recharge Medical releases relevant information about your services needed to secure payment.
  • Prices are subject to change at any time and are never guaranteed.
  • An account is considered delinquent if unpaid after 30 days and will be referred to collections.
  • The current late fee will be applied to unpaid balances.
  • The patient or the patient’s guardian is subject to all service and legal costs related to collections.
  • No services or documents will be provided until the full balance is paid.
  • Recharge Medical accepts only the following payment forms: Visa, Mastercard, American Express, debit cards, and cash in United States currency.

Refunds

  • Charges for services, treatments, and fees are non-refundable and final.
  • When buying a package of treatments, the package price is less than the total price of buying each treatment individually. If a package is not completed, you are entitled to a refund of your original package, less the cost of the individually priced treatment(s) for the session(s) you received.

Communication

  • Recharge Medical and their partners (e.g., Practice Fusion) may contact you via the email you provide, via text message on the cell number you provide, and/or via voicemail on the phone numbers you provide. Recharge Medical may leave voice, text, or email messages regarding issues, such as: appointments, billing, and outstanding requirements.
  • Outside of office visits you may communicate with your physician by way of the medical assistants over the phone. If you want to communicate with your physician directly, make an appointment to do so. You will be charged for the visit.

Prescriptions, orders, and paperwork

  • All prescriptions, refills, referrals, test orders, and paperwork are to be issued only during appointments.
  • Any such needed documents may be requested only at your appointment.
  • Refills, completion of documents, and/or orders must be requested at least 7 business days before they are needed.
  • There may be additional charges for these services.

Controlled substances

  • Prescriptions for any controlled substance are not provided during a first visit.
  • If ongoing prescriptions for controlled substances are needed, the patient will be referred to the appropriate specialist.

Pets

  • In compliance with local health codes and ADA, only certified animals (such as seeing eye dogs) are allowed inside the clinic.
  • Emotional support and other support animals are not allowed at any time.

Patients are responsible for

  • Providing their most current phone, email, and mailing addresses.
  • Providing their most current information concerning any and all changes related to their health, medications, allergies, or treatments.
  • Ensuring Recharge Medical receives all information (e.g., additional tests, vaccines, identification) in order to complete exams, treatments, and/or documents in a timely manner.
  • Informing Recharge Medical of any changes to their payment information.

Feedback and concerns

  • Recharge Medical staff want to know about any concerns about services you receive. In most cases they are able to address any problems encountered. Recharge Medical providers would much rather hear your concerns than have you go unsatisfied with the service you received.
  • If you have concerns about the conduct of any employee, HIPAA compliance, or other regulatory issues, contact the office and ask to speak with the chief operating officer.
