As of May 6, 2024
1. Polio vaccine required
For exams completed on or after May 1, 2024, the applicant must provide documented proof of a full CDC-approved vaccination series for polio. If an applicant cannot provide proof of polio vaccines or their polio vaccines were not given on the CDC-approved schedule, they may complete their I-693 after receiving at least one dose of polio vaccine. Details: Polio Vaccination: What Everyone Should Know. For exams completed before May 1, 2024, polio vaccine was recommended but not required.
2. COVID-19 vaccine required
Applicants who have completed a full primary COVID-19 vaccine series prior to the exam do not require additional doses for immigration if the vaccine series was FDA-approved or authorized for emergency use. If a COVID-19 vaccine dose is due at the time of the exam, the applicant must receive that dose to complete the exam. However, applicants do not have to postpone completion of the I-693 to complete a primary series of the COVID-19 vaccine.
3. I-693 exams do not expire
Any I-693 exam that was properly completed and signed by a civil surgeon on or after November 1, 2023, does not expire and can be used indefinitely as evidence to show that the applicant is not inadmissible on health-related grounds. For exams completed before November 1, 2023, the two-year validity period is still enforced. Details: Policy Alert (PDF).
4. I-693 form edition date
As of May 6, 2024, the I-693 form edition date is 03/09/2023.
5. Electronic reporting
Applicants are still required to send USCIS a completed, signed, and sealed hard copy of the I-693 form. The new electronic reporting requirements are only that civil surgeons must, in addition to the hard copy report, also upload information into the CDC’s eMedical system for all patients that test positive for tuberculosis. The new requirement is in addition to current requirements that all positive tuberculosis results are recorded on the I-693 form and reported to the local health department.
